Introduction
Microsoft Forms is an online survey creator app, a part of the Office 365 Suite. With Microsoft Forms, you can create basic input forms, survey, polls, and quizzes, and invite others to respond to it using almost any web browser or mobile device. You will be able to see real-time results as they're submitted, and export the results to Excel for analysis.

Let’s see how you can find MS Forms in your Office 365 suite. Login to
here.
You should see a Forms app listed as shown below, click on it.
It will open
here in a separate tab and will automatically log in with your Office 365 account. If not, then enter your credentials.
That’s how the Forms homepage looks. Click on the "New Quiz" icon to start creating new.
As shown below, enter the quiz title, description, and then choose the type of response you want for your questions. Once selected, it will provide further UI to enter questions and answers.
There are different types of question responses you can choose – Choice, Text, Rating, Date, Ranking, Likert, Net Promoter Scope. We will see a few of them in details.
First response type - Likert
As shown below, select the question response type as Likert.
That’s how the UI will look for the selected question. Isn’t it amazing? Just one click and you can build such responsive forms for your users. Depending upon your need, you can choose the response type. You can enter the values for which you want a response on the left side.
On the top right, you can specify the response levels. You can delete levels and add more easily. This is a generic example of specifying your proficiency in technical skills.
That’s how it looks when previewed. Very straight forward to create.
Second response type – Net Promoter Score
As shown below, select Net Promoter Score response for one of the questions in your survey or quiz.
That’s how the response UI looks. You can choose this response where you want the users to rate for some service or product etc.
That’s how it looks when previewed. Adding such response types will make surveys more interesting than the regular text boxes.
Third response type – Ranking
Choose Ranking as a question response type as shown below. In this, the user will be able to set ranking to the values provided.
That’s how it looks with sample questions of ranking your office premises from top to bottom. Users can simply drag-drop the values and set it from the top to the bottom ranking.
Responses and analysis
To view responses of your forms, go to forms.office.com. On the homepage, you will see forms created by you or forms shared with you. Click on it and click on the Responses tab.
It shows the count of responses, average or percentage response value for each question, shows average response time taken by users to respond to the form.
Click on three dots ‘…’ to see more options. You can delete all these responses or print the response summary.
By clicking on the "Open in Excel" button, you can open or export all the responses to Excel. Well, this is how the exported response looks in an Excel file.
