upload and feedback

 hiiii

 i am creating a website for job portal. i Have created two menu upload and feedback. In feedback page the user has to provide information like

Name of Person:

Reason: 

Contact no. :

Email ID:

And submit and clear button

When the user clicks the submit button email of feedback should be sent to client whose email id is [email protected]  and  password is abc.

The upload page has a fileupload button for the user to select the resume file and a upload button.

I want to give the limitation that the file to be uploaded should be .doc,.jpeg etc.

and when the  user clicks the upload button a mail must be sent to the client of the file uploaded to the email id [email protected] .

Please provide the code for the upload and feedback.
