submit button pblm...

my problem is that when i have click on submit button then the data shows in the table and as well as showing also in form fields and if i have click again then it create a same ro in table.
i want to show data in table only one time after that if i have click on submit it doesnt show in table with new row. and my loader is not showing...

pls solve my problem..
