Storing database of an HTML form

Hello!

I have created a very basic HTML Form for student record and I want to store its data into the database using PHP.I am using xampp for this. But I am getting an error "cannot POST/ connect.php"... Please help _/\_

 I am also attaching the code that I have written using Brackets editor.
