Start Date and End Date using DateTimePicker JQuery UI MVC

Question

 Hello Friends,

I have one client requirement, where in they need following table format including JQuery UI DateTimePicer, Date format should be "mm/dd/yy",

I am using MVC5, can anybody please help me with this, I am able to do it, for single row, but not with multiple row, can anyone please help me,

I struggle on internet found some examples but not like this.  Plz Plz Plz help me, I want to give demo to client. Thanks in advance.
