Standard Audit Trail and Audit Log

Question

Please I need a sample or Source Code for a Standard Audit Trail and Audit Log. I want to know when each of the User performs CRUD action in each of the module.

1. What action was performed (CRUD)
2. When the action was performed
3. Who performed the action
4. IP Address of the System
5. Tack all the events
6. and so on.

It must be detail enough, and should be on each module.
