SharePoint 2016- Customize form

Question

Hi All,

Need help on below things:

• There is an image when i click on that , it will be redirect to open a form (registration) and once submit it will be saying "Thank you message " and close the form.
• And all items are stored in custom list.
• once item submitted logged in user will get email.
• Any reference please share ASAP.
