Popup in asp .net

hi friends

will u please solve this one

i have a one image button. if i click that button the popup will be generated that is if u want add again this form, also the popup will contain two buttons yes or no buttons if i click yes the page will be generated again ....
please try to solve and give me the code for relvent one
