Populate text fields based on multiple drop down selections

Question

Hi,

I have 5 drop down fields in my Nintex form and based on what the user selects in the 5 fields , the data in 2 other columns which are Multi line text fields should be populated on the form. 

 

The data for this Multi line text field are already entered in the back end list. So every combination of 5 choice fields is unique.
