Modal popup with html control to save data

Question

I have a Bootstrap modal popup with calendar controls , drop-down list & HTML editor . I want to populate this dropdown list with SQL query "Select Id,role from RoleTable" .I want to save all these values of Date , dropdown list & editor in UI , so user can edit & finally save to database .Please suggest
