I can't upload image inside my folder (i.e Website's folder)

Hello,

 

I am trying to upload the image inside my website's folder but i can't perform the task.

 

But the same code which i have written can easily upload the image inside by local file folder .

Here is the path for local file : /imz/healthz/ 

  

please help me.
