I am working on 'appointment booking' application.

Question

'Appoiintment booking system' application. in the first form, i've used 3 combobox  each for labels service provider, names, and timeslot respectively. A datetimepicker control is used for date. Depending on the service provider(such as electrician, plumber, doctor etc), second combobox list- i.e. names should change. A timeslot dropdownlist should show time intervals at runtime(say 9, 9:15, 9:30....).  I need a source code for this form
