How to use asp.net ajax model pop up window?

hi friends,,i m developing online application form for colleges...here before downloading application form user need to submit their Name,Mail id,Ph no..when they click the download link modal pop up window should appear with registeration fiels(as mentioned above)..i m new to Asp ajax..can any one tell the reference / share code...it's the high time to complete my application...help me out quickly..
