How to Print a Printed Form Paper

Hello,

i have a printed form paper like a kind of any admission form
now i have to fill by application which is alrady hold the all student data and we want to print this form
like  form  paper is

Student Name................

so we have to print the student name over ........ area
give me idea how i do this with window form application
