How to pivot bellow table??

I want to convert my table out in as column name to row values are as follows:

 

id code column1 col2 col3 col4 col5

1   1       1 5          12   12   12     12

 

 

nut i want the below output as follws:

 

id code columnaame values 

1   1          column1      15

1     1        Column1      12

1     1        Column1      12 

1      1        Column1     12 

 

 

and so on
