How do I open one form and close another?

Question

Hey everybody, my title is pretty explanatory...but I'm writing a game program and each form represents a different game.  I can get the program to open the second form, but for some reason when it does it brings up a blank form. What am I doing wrong?  And also , I can't figure out how to open a new form and close the first.
