forget password

Question

Hi friends,

i have one requirement that is when i click forget password button it ll display  one window in that window please enter your email like that



i entered email and click submit button.
i want to Receive my user name and password in my e mail address.


and one thing i have  this all data in single table in my data base.

please help any know .
