entity framework , asp.net mvc and sql server

Question

i don't know process of hospital management .please tell process of HM.how to manage all thing in application. 

i want to create a application on hospital management system in mvc 4.please suggest me that what should be to do first in all process  . please advise me how to create hospital management application. i am confuse that what should be first create database or application .
