Display data in a list view

Question

I have a form "form1" which saves customerid, surname, othername,
 sex and phone number into an sql
database. Form1 has got a button "search". When a customer surname is entered
and the "search" button is clicked, I want customerid, surname and othername of
all customers with that surname to display on another form "form2" in a grid
form


Please help me with the steps and the codes.
