Datepicker with ok button and checkbox in a form

Does any person ever implemented a minor change to his datepicker by having a ok button at the bottom? I am looking something along this line and together with checkbox option, when a user select list from the option given and some javascript checking condition if a user select 1 must prevent him to select other option until that one is met. E.g If a user select date, the check option must allow user to choose that option from that list(date) only. Please assist me, thanks.
