Custom Tool Not Visible in Toolbox

I was following the book Wrox Visual C Sharp and was in the chapter about custom tools. I built the custom tool as described and there were no errors. In the book they have you build the tool, add a form, set the form to be the starting control then when you go to add your new custom tool it should show up under a tab in the toolbox. Note they used the word "should" rather than will. It implies that there may be a reason for it not to show up, but they don't go into that. I can't find my custom tool. There is no new tab, it is not anywhere on the list. Is there a setting or something that I am missing? Of course, without the book in hand you don't know what it says beyond what I wrote. But the tool combines a textbox and a label together with some properties and a couple of methods to handle the placement and size of the tool on the user's form. There must be some option or something that I haven't set in my environment.
