collecting data multiple times on the same form

Hi. I have one form to collect inputs (variables) that I want to process and perform calutations on. I can do that okay and get the results I need for that "Instance"
Now I want to use that form again to collect a new set of data and then perform the same set of calculations - for another "Instance".
For each time I want to use my program I do not know how many instances there will be - so I don't just want to create one or two more forms that are the same.
Can someone help please.
