Built Login Control system in Asp.net

Hello Freinds

I need a quick help from yours. i have to built new login control interface with perfect cording(including DB.am using DB 2008 ser).its looks like this 

user name-*********

password- *********

login  

This is the what i want to do and its should windows Form.Please Any one can help this to i really appreciate them.
