Best way form making search window?

Question

Hi friends i want to make this type of search window. In which when i click on the search button then it give result according to the checkboxs which are checked. The problem is that if i want to write the sqlcommand for for select the record from database according earch checkbox combination then there will be more than 100 combination and it will be difficult to do. Then how can i do it in shortest way.
