ASP.NET MVC dropdown STORE IN DB

Question

 

Clinic name is from one table, if i save this form. this form should save in another table. 

both tables are same database.

 

am using asp.net mvc EF.

 

both table has no relationship, is this possible to store in db. i have googled already but am not getting proper solution here. so any links are suggestions appreciated.
