Asking for right solution for the Video and Voice Chat application, problem getting in client machine IP over INTERNET?

Hi all !
I am working in a Video and Voice Chat application in Silverlight, it's working fine in local enviornment but getting problem over Internet [ using in WEB project ]?
Actually, I am using Socket(TCP/IP) for communication, it works fine in local computer IP ex. 192.168.1.12 or something like.(Server and Client both on local computers)
but when I am running in live server on internet it does not identify the Client IP.
How can i solve this problem so that i can communicate over internet to other client ?
(There is also a client side chat server program that is used to open the client port for audio and video. i am running it on client PC, Should i run it on SERVER ?)
Kindly suggest me some good links or reference for getting right solution !
What is the way and what IP would i have to give for right communication ?
I have found no article which explain clear concept of this solution...!
